How do I?

Register as a Patient

We are currently taking on new patients. Patients wishing to register should complete a GMS 1 form, which is available at reception. We look forward to welcoming you and when you join us we would like to offer you a New Patient Medical.

Opt-out of Sharing My Data

Make an Appointment

We are pleased to invite our patients into practice for face-to-face appointments with their GP.  We have pre-bookable appointments available, or you can call for an appointment on the day.  We have appointments available to book each morning and afternoon and you can book these by calling at 8:30am or 2pm for an afternoon appointment.  You can make a request for a telephone consultation on the day.  Requests for home visits will be triaged by a GP to assess whether a visit is required.  Sick note requests can be made over the phone.  

Please let our receptionists know if you are a vulnerable patient.   

Extended access appointments are available via Miriam Medical Centre.  A member of our team can book these appointments for you. 

Requests for medication can be made by completing a prescription request form and placed in the prescription box in our front foyer.  Alternatively, you can email your request to [email protected].

 

Change my Personal Details

If you move or change your name or telephone number please let us know so that we can update our records.

Request a Home Visit

It is usually better to be seen at the surgery where the necessary equipment and resources are available for the best assessment of your condition. Also, the doctor can see four patients in surgery in the time it takes to do a home visit. If you do need a home visit because you are medically unfit to come to the surgery, please try to let us know before 10 am so visits can be organised as efficiently as possible.

Request a Repeat Prescriptions

We do not take requests for repeat prescriptions over the telephone please order by:

  • Putting the request in the box provided at the surgery.
  • Email – the email address is at the top of the repeat slip.
  • Post – if you would like the prescription posting back please provide a stamped addressed envelope.

Submit a Suggestion or Comment

We welcome suggestions for improving our services. These can be made to individual members of staff or in writing. Forms are available at reception.

Register a Complaint

We aim to provide the best service possible, but there may be times when you feel this has not happened. If you feel you have grounds for complaint, please discuss these with the practice manager, when it is hoped any problem can be resolved at once. We operate a Practice Complaints Procedure as part of the NHS system for dealing with complaints.Patient Liaison Service (PALS) – 0800 054 2137.

You can discuss any concerns you have verbally either over the phone or face to face with our Practice Manager. If you prefer, you can submit a complaint form (attached) or email the Practice Manager directly at [email protected].
